Abstract

Background: Recurrent Clostridium difficile infection (RCDI) continues to increase in incidence and severity around the world. Fecal microbiota transplantation (FMT) is commonly used to treat RCDI in adults, but its efficacy and safety in the pediatric population are infrequently studied. Here, we performed a systematic review to evaluate the efficacy and safety of FMT for RCDI in children. Methods: Systematic literature search was performed using PubMed, Science Direct, Nature, Cochrane Library, and Springer to identify original studies in English language published from 2010-2020. The clinical resolution, bacterial resolution, and failure of therapy were calculated as indicators of efficacy of FMT. Adverse events were also calculated as indicators of FMT safety. Results: A total of 21 studies, with 498 pediatric patients were included in this review. There were 418/498 (84%) patients reported clinical and bacterial resolution. There were 67/498 (13%) patients who experienced FMT-related adverse events, with 97% of them had mild adverse event. There was one patient who reported FMT-related serious adverse events which were aspiration pneumonia and dehydration. Conclusion: We found evidence that supports the efficacy and safety of the use of FMT for treatment of recurrent C. difficile infection in children.

Abstract

Background. Ultrasound evaluation on the median nerve in carpal tunnel syndrome (CTS) patients by measuring cross-sectional area (CSA) is widely known. However, studies have shown that CSA of the median nerve could vary in different demographical factors. Therefore, the cut-off CSA value for CTS in the Indonesian population could be significantly different. Purpose. To compare mean CSA of the median nerve between normal and CTS populations and establish a cut-off point of CSA to diagnose CTS. Materials and Methods. A comparative cross-sectional study was conducted involving 40 people divided equally into CTS and normal groups. Normal and CTS subjects are determined by Electrodiagnostic study (EDx) using EMG and Nerve Conduction Study (NCS). The CSA measurement was performed at the level of the carpal tunnel inlet. Results. The mean CSA value at the level of tunnel inlet of the normal group was 8.3 ± 1.4 mm2 and for the CTS group is 15.4 ± 4.4 mm2. The receiver operating characteristics (ROC) analysis showed 10.6 mm2 as the cut-off point with 95% sensitivity and 95% specificity (P<0.001). Conclusion. The median nerve CSA at the tunnel inlet is significantly larger in the CTS population and the cut-off point can be recommended in detecting CTS in the Indonesian population.

Abstract

Introduction: Muscles are the largest tissue group in the body and can be classified into various classifications, one of which is based on their morphological shape. Examination of muscles, especially skeletal muscles, can use electromyography. The purpose of this study was to determine whether muscles that have similar morphology have no different electromyogram.Method: This is a descriptive observational study using needle electromyography. The sample amounted to 5 with five times MUP (Motor Unit Potential) in each musculus deltoideus and musculusResult: The mean amplitude, duration, number of voltage phases and the number of voltage turns off the musculus deltoideus and musculus gluteus medius in the study subjects were similar (p-value > 0.05).Conclusion: Muscles that have similar morphology in each parameter of amplitude, duration, number of voltage phases, and number of voltage turns do not have a significant difference in electromyogram.

Abstract

Introduction. COVID-19 patient not only experienced respiratory manifestation, but they also generate generalized muscle weakness. This manifestation results in deterioration of physical capacity. Methods. This cross-sectional study identified the characters of COVID-19 patient based on their age, gender, severity of disease and whether they have comorbidities or not. This study also described the functional capacity using a modified 30-Second Sit-to-Stand Test (m30STS) and chest expansion measured at three levels (axillary, nipple, and xiphisternum). Result. M30STS was one of reliable measure that used for physical or functional capacity. Chest wall expansion determines lung volume and functional capacity as well. Eighty-five subjects in isolation ward were enrolled. This study shows there is reduction of functional capacity in COVID-19 patients. Majority of subjects tend to reached lower number of repetitions in m30STS or about 10.37 times in average. Clonclusion. The thoracic expansion was reduced below normal range in all subjects.

Abstract

Introduction: Cataract in workers need to be distinguished whether it is an occupational disease or not. Palm oil plantation workers are one of the high-risk jobs for potential hazards in the workplace that have an impact on health, one of which is cataract. Case report: A 38-year-old male oil palm plantation worker with complaints of blurred eyes since 1.5 years ago, sensitivity with lights and glare, and clouded. He was exposed ultraviolet (UV) rays 5 hours every day for 18 years without UV protective eyewear. To on examination revealed visual acuity in the right eye was 6/60 and the left eye was 6/9, both posterior subcapsular lenses opacities, vitreous papillae on the right were blurred.Discussion: Occupational cataract in oil palm plantation workers can be caused by prolonged exposure of UV rays, without wearing UV protective eyewear. The examination revealed decreased vision, posterior subcapsular lens opacities and blurred papillae. Determination of occupational cataract refers to the Seven Steps Occupational Diagnosis method issued by PERDOKI (Indonesian Association of Occupational Medicine Specialists). Conclusion: Occupational cataract can be diagnosed by the Seven Steps Occupational Diagnosis method and prevented by administrative controls and the use of UV protective eyewear.

Abstract

Coronavirus disease (COVID-19) caused by infection of severe respiratory syndrome coronavirus 2 (SARS-CoV-2) was first detected in December 2019. Since then, many countries have started reporting a series of cases in children post COVID-19 infection, with various clinical manifestations, i.e: persistent fever, cardiovascular shock, and hyperinflammation similar with Kawasaki disease and/or toxic shock syndrome. The terminology Pediatric Inflammatory Multisystem Syndrome Temporarily Associated with SARS-CoV-2 (PIMS-TS) was given by the Royal College of Paediatrics and Child Health (RCPH), hence commonly known as Multisystem Inflammatory Syndrome in Children (MIS-C) as named by the Centers for Disease Control (CDC) and the World Health Organization (WHO). Multisystem Inflammatory Syndrome in Children has a very wide spectrum of clinical signs and symptoms, with the most common symptoms reported is persistant fever, along with gastrointestinal, cardiovascular and dermatological symptoms. Fever is reported in 97-100% of patients. Gastrointestinal symptoms are the second most common symptom occurring in 70% of patients. Until now, the pathogenesis underlying the occurrence of MIS-C is still unknown. Multisystem Inflammatory Syndrome in Children case management requires multidisciplinary knowledge. Consensus or standard guideline for the management of MIS-C is a work in progress, but in general, the initial management includes intravenous immunoglobulin (IVIG) and corticosteroids.
